Our 3 year old is receiving some services from our school sytsem -- specifically a hearing evaluation and some speech services. They have requested a copy of his birth certificate and his immunization record. He actually HAS been vax'd (before I knew better) so I have one, BUT I am not thinking I should turn it into them since I will be claiming religious exemption for all of my children. What should I use in lieu?

Submit only the religious exemption. Check 909shot.com.

Yes, I am good with just submitting a religious exemption -- was just wondering if there was a form that would be best. Checked out that website and did not see one. I'm actually good w/ my decision not to vax, just looking for a form. TIA!

You may want to search under

"Your state's name" vaccination exemption forms

Depending where you live you may have to request them directly from the state and have them notarized and THEN turn them in.
