My insurance says I need to provide a "confirmation of birth" within 30 days of having my baby in order to obtain coverage. Hospitals here in Georgia give these out, but I've never seen one and don't even know what's on it. If I could get a glance at one, I could tell my midwife what it has on it, and she could make one up for me. Hopefully, it would be deemed suitable and my insurance would accept my baby!
That sounds like the thing I need! But I am doing a home birth, so I won't get a form. I asked my insurance, and they said there is a specific form that all the hospitals use, and that I need that form in order for them to add the baby! Do you still have yours, or a copy of it, and is there any way you could scan it in so I could see it, or take a picture of it? I want to format it correctly so that it has all the right things so my insurance will honor it.
