Hi momma's! I hope it is ok to ask here rather than in the vax area but I am wondering if there are any non vaxing moms around for PA? I need a legitimate exemption form. I tried google and am just finding articles on how they can be incriminating, which I do not need!
Anyone?

I don't know that there is an official form. I always just wrote out:
I object to vaccinations on the basis of religious grounds.

To Whom It May Concern:

Re: Religious Exemption, CHILD NAME

We, XXXX (Parents names), being the legal guardians of XXXXX, object to the vaccination requirements as outlined by the Department of Health of the Commonwealth of Pennsylvania as defined by 27.PA.CODE CH. 77 on the basis of a strong moral or ethical conviction similar to a religious belief as outlined in section §23.84 Exemption from immunization. On that basis, XXXXX will receive no further vaccinations.

This is what I just used for daycare and they had no issue with it.

The state health department physical exam form (which you can find on AskPauline) has a box to check. I don't do more than that unless I am explicitly asked. If asked, I do no more than the letter above. Restate the law. Done. Never had a problem.

Oh and I also wanted to add the reason I said "will receive no further vaccinations" is because my oldest had a few before I was educated and dummy DH signed the form at the hospital while I was sleeping to give my youngest the Hep B. The health form we had didn't have a box to check, but instead had an area to fill in what they had. The doc just made photocopies of what he had on his files and attached it to the health form and then signed it for me.

Our daycare is great and she actually said she has 4 other kids with no vaxes and 2 other kids who are delayed in the daycare and is considering not vaxing the one she is pregnant with right now since her youngest had a bad reaction to his second dose of MMR (he is 10 years old) and there are so many more now days that are reqiured then when her other kids (19-10 years old) were young. Hopefully school goes this easy next year when my oldest starts kindergarten.
