Vaccine exepmtion for YMCA?

Hi Everyone,
I was interested in sending my son to a program at the Y. For registration, they will need a medical records form showing his vaccinations, but he is not vaccinated yet. Has anyone else experienced this at a Y in NYC? Will I have to first go through the exemption process or can I just check a box on the YMCA form that says we haven't done any because of religious reasons?
Thanks for any help with this!

Either,

1. move
2. Check the box.

NYS is a virtual mess when it comes to vaccines and exemptions. They have a law that grants exemptions, and then they do not allow you to use them. There are attorneys who make $$ writing letters, but the law is the law, and I do not know why the state does not recognize its own law. It has been that way for over 40 yrs.

I was never vaccinated and I belonged to the YMCA for years. But I am not in NYS.
