This is actually for me! I'm being required by work (via the health department) to get the hep a vaccine because my job is in food handling. I understand from reading the code that I can claim religious exemption, but I can't find any information about HOW to do that. They don't make finding a form or information on how to claim the exemption obvious. I've found stuff regarding kids and exemption for school, but nothing for adults so far. Anyone have any information?

Whenever I've needed an exemption form with my kids, it was always provided by the entity which needed it. If your employer doesn't have it available, I would call the health department and ask where/how to get one.
