Xerxella Check your state's laws as others have stated. But, in Illinois, the doc just has to sign the medical form stating which vaccines have been given out and when, etc. Ours ends up blank and then the doc signs that agreeing that no vaxes have been given. I write the exemption letter and include that with my paperwork.

BlessedOne Maybe it is different where you are, but here we don't need a dr. signature. My first exemption forms, I just went to the local health dept and they made me read a long paper about vax that was supposed to change my mind, but made me just be more against it. I had to initial the paper on the front and back and then fill out the card with child's name and check all vax I wanted exemption from and then check one of the reason why I wanted to be exempt and sign. They told me to keep the card like I would my vax record. When my dd started school, that is what I showed the school. Eventually the school wanted a new one every year, but instead of going to the health dept, they had the cards in the nurses office and now every year I just refill out. I never need any dr signature or anything. Although perhaps the nurse would sign it later, as she would keep the card. Either way, I didn't have to go to the dr for anything.

I am not in the same state, but you might check and see if you have to have the signature or not. I am sure it probably varies from state to state....but it is worth checking into =)

applejuice I recommend that you find a family doctor, naturopath, homeopath, or some other practitioner who can sign off - check your laws to see who can sign ...

Call ahead first and make sure these doctors still feel the same way about vaccines as when they were put on these lists.

Looking for Doctor willing to sign exemption forms

I live in western Kansas and the Dr that I went to to get the exemption forms signed for school, has died. Now I`m trying to find another Dr. that is willing to sign those forms. I`m willing to drive several hours if I have to, but just need an idea of where to look and who to call. Can anyone help me out?!
