DS, 6, suddenly won’t wear underwear

Please help! He says he still feels wet after he has peed and wiped, and then he doesn’t want to put on underwear. He emphatically says he is so uncomfortable. He will only wear silky nylon shorts or a too-big pair of sweats. He is intact and we took him to the doctor. She said it could be balanitis and to use bacitracin for 7-14 days. We did. DS said it made it worse. He doesn’t seem to have any infection. We have bigger underwear on order, but I don’t know if it will help. I thought about sensory issues, but he doesn’t have any other sensitivities. He is so upset. A friend said to just let him go commando until the fall, but we’re afraid the behavior will become entrenched. Ideas? Anyone seen this? I thought it might a glans separation thing, but it’s been going on for months.

"Behavior become entrenched"? Huh? Like, not wearing underwear forever? So what. I promise you, a free-balling kiddo is not a parenting hill you wanna die on. Bigger and badder things are to come. Save your energy for the big stuff. Let the kid go commando. NBD, I promise.

When my DS had balanitis, there was little doubt. The area was visibly inflamed and he was in a lot of pain. The medication cleared it up within days. Maybe something else is causing the irritation? Could it possibly be the way he wipes?
