So... We use Avent pacifiers... Wasn't intending to ever use a soother, but there you have it.

I always worry about the silicone nub coming off and him choking... Anyone else terrified of this? We test them and such, but I would just like to know if anyone knows of anyone that has choked on a pacifier nipple?

We used lots of different kinds of pacifiers with dd (she was a total "suckie" addict--lots of issues that led to that; it's a long story). Anyway, she could be pretty tough on them, she used them until she was 3.5, and she would sometimes actually bite on them and pull as hard as she could, just to play. They never came apart. So no, I'm not worried about it.

If you read the package it says to replace them every 4 months or something like that? I think if you do that, you wouldn't have much chance of a problem.

I know the AAP recomends one piece paci's because of that risk, but I've never actually heard of it happening. I agree that if you replace it often you are probably Ok.

I don't know. I've never heard of that. We have used Avent & Nuk.
