If you want a pouch per se, then check out Kangaroo Korner adjustable pouches, Mamma's Milk invisibly adjustable pouches, and the Tentoes adjustable pouch. As far as I know, all of these have to be adjusted before you put them on and before you put baby in, so there's no adjusting on the go.

IMO/IME, a ring sling is really not very difficult to learn to use at all, and gives you the quickness/convenience of the pouch plus adjustability on the fly, which I think is awesome. An unpadded or only lightly padded ring sling gives you all the positions you can do in the pouch, but is so much easier to get a custom fit with. My favorites are the Maya Wrap lightly padded (NOT original), EllaRoo, and Sleeping Baby ring slings.

I have an adjustable pouch made by Maya wrap. I LOVE it! It is by far my most favorite carrier I use out of all of mine. I have used it with ds from birth to now (he is two and a big boy).
It has zippers to adjust the length that are covered with flaps that button. Sounds confusing, but very easy and not bulky or uncomfortable. I actually found it used here on the trading post.
maya adjustable pouch

I love my Bundle Of Love (www.bundleoflove.net) pouch with the Aplix snaps for adjustability. I have the chocolate cotton stretch and it's so pretty. My ds is getting a bit too heavy for me on one shoulder, but I can't wait to use it with a newborn.
