DD is 27 mos and we just moved her to a twin bed. We have the Dex bed rail and the other side of the bed is against the wall. She sleeps like a fish....all over the palce, and has never fallen out. The bedrail is pretty long, but leaves enough room that she can get out of bed herself at the foot end (about 18" or so). Also, the rail folds down in the daytime so I can make her bed and the comforter covers up most of it.
Check it out

I'd wanna put his bed next to ours and if I only do mattress there will be a big gap because our bed is very high.

To address the thread title question directly: Never.

I have fallen out of bed while sleeping as an adult.
:

No, I wasn't drunk. Of course, I still occasionally end up with my head at the bottom of the bed and feet at the top, too.

:LOL :LOL :LOL

I guess I could have chosen different title for this thread. :LOL

I think last time I fell of the bed was when I was about 8 years old and it was olny because I slept in unfamiliar place.

I think it really does depend on the kid (or adult :LOL ). We've had DD in a toddler bed without a rail since she was 22ish months old, and she's never fallen out of bed. Of course, we co-slept with her for well over a year, and never had a problem with her falling out of our bed, either, so we were pretty sure she'd be fine.

You could try starting out with a mattress on the floor, to see how he does.
