I suppose it depends on what the child is into, really. Big hits for my 3 yo:

Play fabrics to be used as tents, dress-up, blankets, etc.
Schleich animals
Farm set with homemade playmat
Spices to make "soup" with in water
Stuffed animals
Wooden beads (she uses these as play food for her stuffed cat)
Play kitchen supplies: The biggest hit at our house is the spice rack that I filled with containers of dry pasta and beans to use as her "ingredients"
Chin-up bar for doorway (dd is a natural climber!)

I try to buy things that aren't "age specific" because I have no desire to buy lots of new things each year.

A collection of dress-up clothes from goodwill (not things specifically made for dress-up, but a range of fun clothes my aunt found for us). 5 years later, they're still in use.

We bought this for DS2's # 3 http://www.walmart.com/catalog/produ...uct_id=4938645 he really liked it. It might seem extravagant but I literally spend $20 on parties so I pass the savings on to my kid's gifts, which is usually just one large gift and whenever possible I ask relatives to pitch in rather than to buy a gift separate.

We got my 3yo a bunch of Schleich animals for his bday and he LOVES them.
He also likes Legos, Playmobil, his Leapster2. He got a bike when he was 2 but that would be a must if you don't already have one.

I'd suggest http://webkinzjr.com . It's a toy and your lo can also play on their website with your help. I haven't check it out by myself but I believe it has the same good quality of Webkinz. My daughter collects them. The site is really great and safe for kids. I had to set a time limit because she'd play for hours!

My dd's favorite third birthday gift was a set of Tupperware play dishes with a real working pitcher and tumblers.
