Has anyone homebirthed in NYC before? Did your midwife provide the paperwork necessary for obtaining a birth certificate and SS#? Or did you have to do it yourself? I see my back-up midwife on Tuesday (our's is on vacation) so I will ask her, but I wanted to see what everyone's experience with it was and how long it took to get both. (I need it to get the little one on health insurance- long story.) Thanks!

Our baby was born at home in NYC in April. Our midwife provided the form (one form for both ss and birth certificate). My husband filled it out and she signed it and sent it in. There was some kind of mix up and it took 2+ months for us to get them but it is usually much faster - like 6 weeks or less. And to our surprise, when we got the birth certificate, it was in my husband's hand. They must scan the form and use part of it for the birth certificate. That was pretty cool.
