We are flying tomorrow on a 45 min flight, yes it is short, but 45 min of constant screaming can feel like an eternity. We have done this flight twice before, and my ds screamed almost the entire time. It was awful. I managed to get him to sleep on one leg by bouncing him in my seat fairly hard. I think everyone thought I was nuts. But hey it worked.
He is fairly high needs, and when tired wants to lie down and nurse and nothing else will do. I tried nursing in the seat, but no go, he hated it. He won't nurse unless we are side lying or i am bouncing him in the sling. Neither of which will do on an airplane :LOL And I think his ears bothered him on the ascent and descent, as that is when he would cry the most. Now I am dreading this flight, and I am hoping i can distract him. Any ideas? He likes anything musical. Like street light crossing signals could entertain him for hours.

Will he drink out of a sippy? When my DS was that age he didn't do it enough so that it was a source of hydration or nutrition, but it was more for fun. I would just put water in there. Or in your situation I might even do juice. I am normally against putting juice in sippies (have never done it) but if I thought my child was going to be in pain for 45 minutes I might try it for that situation only.
