just checked mine out online- for those who may have Blue Cross Blue Shield Florida "State Employee" coverage:
Payable Network - 80%
Payable Non-Network - 60%
Special Limits/Circumstances -
Fertility Testing and Treatment Services:
Some fertility tests and/or treatments are considered investigational or experimental and are not covered.
Artificial insemination, gamete intrafallopian transfer, ovum or embryo placement or transfer, in-vitro fertilization, cryogenic and/or other preservation techniques used in such and/or similar procedures are not covered.
of course, hoping I won't need that info myself (and no one else here of course!), but sharing just in case anyone else might need to know.....
