OK, for those of you don't know our back story, here is a summary, others can skip to the next paragraph. DD had a near-SIDS event at 4 months of age and has a profound anoxic brain injury. SHe is delayed across the board. Due to decreased ability to swallow or maintain her airyway, she had a trach & g-tube (no fundo) placed 8/20/09.<br><br>
Alyssa is usually very happy & healthy. She got a bad resp virus athat had us in the hospital from 5/1 - 5/8. She has been doing well without any oxygen and off nebs (back to baseline). Two weeks ago she spiked a temp, and we took her to the Dr. who said she had an ear infection, so we started 10 days of abx.<br><br>
The past 5 days she has been getting very upset, crying, increased tone, increased heart rate. Her ears are clear (she got a clean bill of health sat). She is fine when she is asleep, the pain/discomfort seems to come & go. Neither of our diets have changed (she is tube fed foods I make for her and EBM). Her tummy is soft, and her poops are normal. I can not see any teeth. Until today, the episodes were infrequent, and resolved with Tylenol or Motrin. Today she has been upset all day, does not want to be held, her heartrate is high, the tylenol & motrin do not seem to do anything. She even cried through a bath (which she usually loves).<br><br>
Any thoughts? Suggestions? We will call the doc tomorrow morning (her specialist does not have anyone on call), but I would like to see if I can come up with anything that might help.

Thrush/yeast from the abx?<br><br>
Sinus pressure from impending teeth/change in weather??<br><br>

No thrush or yeast.<br><br>
I will have them check for UTI tomorrow, it had crossed my mind, but she does not have a fever.<br><br>
No on the sinus pressure, because her secretions have not increased (we suction mouth & nose as well as trach.<br><br>
She has 4 teeth in, but I cn neither feel nor see any more. That was my thought when the episodes first started, but her other teeth came in fairly quickly once she was uncomfortable.<br><br>

what are her hips like? Could one of them have slipped out? Anything else along those lines? I hate it when you don't know what's causing them pain.

her hips are fine. Her range of motion is good, when she is not being hypertonic. She also has not been flinching during PT when we start it before one of these episodes.

When my DD was like that for a couple of weeks it ended up being a precursor to seizures. Infantile Spasms to be exact. Her brain waves had changed and were disorganized and this made her inconsolable. We thought it was teeth... but a week or so later she manifested spasms. I think your DD is a little old for onset, it is usually between 4-9 months. If you can't come up with anything you might ask for an EEG.

Alyssa has seizures, but is controlled on Keppra.<br><br>
She still had a bad episode late this afternoon, but she is happy right now (after Motrin). The doc looked at her and said his best guess was a virus that had not manifested yet (i.e. aches & pains, but no other symptoms yet) or teeth that have not broken yet.<br><br>
We are going to keep treating her with Tylenol & motrin, and have an appointment with our physical medicin & rehab doc on Friday. Our neuro had us up the dose of Keppra just in case.
