I can't provide all the information you're looking for, but I'll do what I can!
We're not *required* to vax, as home or bricks and mortar schoolers. Most provinces have rules that allow exclusion of unvaccinated children from schools during times of 'outbreaks' - which can and has been interpreted to mean everything from a couple of incidences on up.
A few provinces do require that parents submit paperwork stating that they are refusing to vaccinate, but I don't know anything about this. I *think* that New Brunswick and Ontario do this, but I'm not sure.
I don't have a kid in the public schools, but the swine flu vaccine wouldn't have had a law of its own. So I imagine that schools had the right to exclude unvaccinated children at the height of the outbreak.
I've never tried to register a child for a brick and mortar school, so I'm not sure what vaccination paperwork is required. I have registered my 6 year old (AB monitors but also provides $) as a homeschooler and there was not a single question asked about vaccination.
Here, the vaccinations are administered by the public health unit, not the doctor's office. So the parent is responsible for calling to arrange it. If a child is known to teh system (ie born at the hospital, etc) then a vaccination reminder card will be sent periodically but no one follows up with it. Even my doctor doesn't tend to ask about vaccinations, assuming that I've been taking the kids to the health unit.
