All the Googling I've done tells me how to apply for a copy. I cannot seem to find anything at the Health Department, Center for Vital Statistics or anywhere about what is required to apply for a birth certificate. Anyone have a good website?

I have found that you really need to call and ask in person. It's not something they really acknowledge on the net.

i just took the baby with me. i had ID and filled out the information. there was an application form which you need to get so that both you and dh can sign it (if you want his name there...just assuming here).
i took that application (could NOT get it online..i received it from my MW but SHE got it from the county clerk's office).
It had to be done in the town of birht (which for me was more than an hour away so i got it the day before he turned 6 months. apparently i had to get it by law by 6 months! nothing like cutting it close).
