DD always had a spot I couldn't reach. Just do the best you can and make sure you clean the whole mouth, not just the tongue.
You can also get some GSE (grapefruit seed extract) and use that in conjunction with the nystatin. Clean her mouth out after every nursing with the GSE and water and use the nystatin as directed.
The directions on my nystatin for DD says to give her x amount but I know from my LC's that it's most effective if you 'paint' the mouth with a good amount of it. Be sure to get the gums and the inside of the lips too.
ETA: you may want to consider diflucan for yourself. Yeast is often internal on mom and ointment won't help that. Plus trace amounts of the diflucan make it to DD through your milk, which can help her healing process. Do you have her on probiotics too?
