I would look closely at positioning. I get irritated when Luthien's position si bad because it messes up her latch. Also, she tends to clamp down when she falls asleep, thus applying needle-sharp pressure where her teeth are. For that I just break suction and either she stays asleep or re-latches and sucks some more.

I've seen good advice on extended breastfeeding to make sure that baby's mouth is clean before nursing, have him take a sip of water or some such. Food can irritate.
