You know, I've been tempted to send out a cover I didn't know for sure had been washed before its last use, but I decided to do a super thorough check of it first and found a teensy dot of orange bf poop on a binding! I was so glad I found it and washed it. That was a lesson to me to always wash before sending something out.
Conversely, I have been on the receiving end where I got a cover that had strong ammonia from pee in it and dust and dirt and bits of bark all over the bottom. It was so obvious this cover needed washing before it was sent but I have to think it had to be an oversight from a weary, distracted mama. So I usually wash anything I get now.

I'm assuming it must have been an oversight. It might have even been washed - it took quite a bit of work to get the poop off the cover (had to soak it and then scrub it). I contacted Heather for an opinion of how to handle it since I don't want to go on the attack in the feedback forum with something that the mama in question would most likely be horrified about.
