Ok, what does: "A physician’s note is required for medical exemptions, and a practitioner’s note is required for religious exemptions."
So if I use a religious exemption I need to have a religious practitioner sign it?
Anyone dealt with this...
My son is not eligible for a medical exemption.... febrile seizures would not be at all close enough to even try, obviously, and I was just learning at the time, so I didn't connect the 2 events till afterward. Even if I wanted to try, I don't have the documentation.
I know we are toeing the line for religious exemption (as we don't align with any particular one religion) but I know there are several religions that take issue with one, some or even all vaxes.

Yup, I found the wording here and used it and had no problem. Also, I thought they couldn't ask you which religion you were affiliated with?

It's in a school handbook, public school- one of 2 places we are likely moving to (fingers/toes crossed waitning to hear from brokers).
I was thinking of just using my standard letter, stating the law and making them ask me, but I'm wondering if anyone has 'had' to get some practitioner signature.

So I decided to find this all in the MGL's:
http://www.mass.gov/Eeohhs2/docs/dph/regs/105cmr220.pdf
220.500: Immunization Requirements for Preschool, Elementary, Middle and High School Students
(C) The requirements in 105 CMR 220.500 (A) and (B) shall not apply:
(1) upon presentation of written documentation that the student meets the standards for medical or religious exemption set forth in M.G.L. c. 76, ss 15;

And here is MGL 76, 15 (relevant portions)
http://www.mass.gov/legis/laws/mgl/76-15.htm
CHAPTER 76. SCHOOL ATTENDANCE
Chapter 76: Section 15. Vaccination and immunization
Section 15. No child shall, except as hereinafter provided....

In the absence of an emergency or epidemic of disease declared by the department of public health, no child whose parent or guardian states in writing that vaccination or immunization conflicts with his sincere religious beliefs shall be required to present said physician’s certificate in order to be admitted to school.

Also, if I look here: http://www.vaclib.org/exempt/massachusetts.htm
under school aged, well it is going over my head a little right now because they use the term 'certification' .... in any case they reference a legal case....
Exceptions: MGL 76, &15.
Exemption does not require the objecting persons to be a member of a recognized church or religious denomination. Dallo v. Board of Education, 358 Mass 753, 267 NE 2d 219[/B]

220.500: Immunization Requirements for Preschool, Elementary, Middle and High School Students

Jessica

BTW, this is a template shared here years ago that we adjusted and have been using- it is not the school-aged one, so I'll update it with the relevant portions above...

To Whom It May Concern:

Statement of Exemption:
I hereby claim exemption for my child, name, from the Massachusetts immunization requirements. I am exempting my child from vaccination because it conflicts with my sincerely held religious belief.

Regulations Of The Department of Early Education and Care
102 CMR 7.00: Standards For The Licensure Or Approval Of Group Day Care And School Age Child Care Programs (7/10/97)
Section 102 CMR 7.09(5)(a) Required Medical Examinations in Group Child Care Programs

“The licensee shall require, at admission, a physician’s certificate that each child has been successfully immunized in accordance with the current Department of Public Health’s recommended schedules against diphtheria, tetanus, pertussis (whooping cough), poliomyelitis, measles and such other communicable diseases as may be specified from time to time by the Office. No child shall be required, under 102 CMR 7.00 to have any such immunization if his parent(s) objects thereto, in writing, on the grounds that it conflicts with their religious beliefs or if the child’s physician submits documentation that such a procedure is contraindicated.”

if you're concerned, there's also a place online where you can get ordained as a minister (or someone you know could, and they could sign the form). it's the Universal Life Church.
here's from their website (themonastery.org):
"The Universal Life Church ordains clergy for life for free.

Each denomination has its own special requirements that its ministers comply with or perform to gain their allegiance. The Universal Life Church is the only religious and spiritual denomination in the world that opens its door to all and welcomes all who ask to get ordained, granting ordination without questions as to religious and spiritual beliefs."

good luck!

The connection died out on me last night when I replied.... My dh never really was sold on Scituate because it is farther away from the highway. We are going to be in S Hingham just around Accord Pond. So excited. Exhausted. We've done a lot the last week but everything seems to be settled now. I think I'm still hyperventilating....
That's still really close- so yea! And I won't be too far from the gang farther south!! Yeah!

Jessica,

Definitely don't worry about it...we're never had an issue. This is the version we use:

To Whom It May Concern:

My child, [name], will be attending [school] for the 2009-2010 school year. I am providing you with this statement in order to satisfy state immunization requirements:

In accordance with Department of Education and Early Childcare Regulation 102 CMR 7.00, the administration of immunizations conflicts with my sincerely held religious belief.

Sincerely,

(Personally, I like this version because I feel less like I am lying

If it's where I'm thinking it is, I LOVE that neighborhood! And you're right near Not Your Average Joe's when you don't feel like cooking

LOL, I havent eaten at that location but years ago at another place, and it _was_ average, we were so disapointed, but hopefully it was just a bum night. I'm excited that it is so close to Whole Foods.
Hopefully we'll catch you tomorrow at the Farmers' Market.
