i know that for school, you need to fill out the affidavit form to be exempt from vaxes. is it the same for preschool/daycare?

On your application there should be a place for vaccination information. At the daycare I used to work at, people just wrote something like "not vaccinated" or "delaying until age whatever". Keep in mind that vaccines in Canada are recommended, not required.

You can download a form from your local health unit. I am a home daycare provider and my kids also attend school and the form is the same. In Canada you can not be forced or excluded based on vax. Of course, in my daycare I couldn't care less if your child was 100% pure and natural!<img alt="" class="inlineimg" src="http://www.mothering.com/discussions/images/smilies/lol.gif" style="border:0px solid;" title="lol"><br><br>
I never ask for vax records.
