Hi! I don't post much on this board, but I do lurk occasionally
I am in distance ed grad school, and have to be on campus (and out of state) for about 9 weeks this summer and next. My regular sitter is a young woman we have known since she was a baby
who stays here off and on anyway and has for years, and will be moving in full time while I am gone this summer. Dh will be working full time and home in the evenings but he works out of town some days, and I need to make arrangements for our sitter to take the kids to the dr if sick or there is an accident or something. My youngest has some special needs, and I can't imagine him going the whole summer without at least one visit
Dh and I can be reached by phone, but he will often be several hours away. What paperwork do I need to line up for our sitter to be able to get them medical treatment? I am sure my ped would see them as is, since I often take the sitter to appointments with us (she likes to know what is going on, and it makes care of the baby's specific needs easier), or with just a phone call. But what about the urgent care clinic and ER? Thanks!

For all the kids in my care, their parents fill this out and have it notarized along with a copy (front and back) of their insurance card.

AUTHORIZATION TO TREAT A MINOR
I, the undersigned parent of _______________________ a minor, do hereby authorize and consent to any x-ray examination, anesthetic, medical or surgical diagnosis rendered under the general or special supervision of any member of the medical staff and emergency room staff licensed under the provisions of the Medicine Practice Act , of a Dentist licensed under the provisions of the Dental Practice Act, and on the staff of any acute general hospital holding a current license to operate a hospital from the State Department of Public Health. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required but is given to provide authority and power to render care which the aforementioned physician in the exercise of his best judgment may deem advisable. It is understood that effort shall be made to contact the undersigned prior to rendering treatment to the patient, but that any of the above treatment will not be withheld if the undersigned cannot be reached.

Mystic Momma's thing sounds good - the only thing I'd add is to ask your pediatrician what s/he recommends. I worry that MM's form may exclude treatment and advice for your child's special needs, and you want to make sure that those things are covered.

MeepyCat, my plan was to give a copy to the sitter, a close friend, and file copies in each of the doctors' offices, so hopefully if there is a question they can address it ahead of time with me. I have been able to arrange most of the baby's stuff for before and after I am gone (like his swallow study and allergist stuff), but I am sure that something will come up
Also, we may be starting feeding therapy over the summer, so there would be weekly visits for that. I also intend to type of medical histories on all three of them, so she could just hand it to anyone at the hospital or whatever.

The closer leaving gets, the more complicated turning my life over to someone else seems to me
