Okay, I need some thoughts here...a reality check if you will. Okay, dd is 3 and will be 4 end of Oct. She's been potty trained since she was about 2. She did it on her own. No real push from me at all. She just up and decided one day to use the potty. She had the typical accidents for a while. I'm not sure how long, but maybe 6 months (?). But honestly she learned it pretty quickly. I must admit I was relieved that it was so "easy." Here in the last couple of months she is having accidents where before there were none. I would say a couple a week? They are always at home. She's more attentive out and about.

My other question is, at what age, generally, do children achieve nighttime continence? That she achieved for 2 weeks when she was 3 and never since then...

I don't know about nighttime. I have a daughter who just turned four and she didn't have an accident for over a year and then all of a sudden she started having episodes where she'll just slightly pee in her panties. I think she feels like she can hold it for longer than she really can. If I catch her looking like she might pee, I'll ask her to go. She'll usually refuse and then request to go herself a few minutes later.
