I've done it. It was no problem, just make sure you get it done before the baby is 1 year old. My dh's name was filled in the " birth attendant" space. They didn't hassle me or ask questions, just did it and were very nice and courteous. They just sent me the paperwork, I filled it out and sent it back in and they sent me the birth certificate. Oh, and I live in so. utah, so it'd probably be faster for you, being that you could go right in and do it!

Sarah, Did you get the forms from the Health Department or the Department of Vital Statistics? If it was the Health Dept. did you go through the County or the State?

I truly appreciate all of your help!

Hmmm... I'm pretty sure it was the department of vital records. My health department here didn't have the paperwork or the power to do birth certificates, so they had me call the state. Basically I just called the number in the gov. pages for the birth and death certificates, they transferred me to the person who handles homebirths, she asked who the midwife was, I said there wasn't one... They then sent me the correct paperwork, with my dh's name already filled in as the attendant. I just filled out the forms, sent it back in with a $15 check, and about 6 weeks later I received the birth certificate.

I could tell by her tone of voice, after I mentioned the no midwife thing, that she was a bit surprised...but she never said a single thing about it. Thank goodness too, I'm not good with confrontations.
