I took dd(6yo) to new doc today. They wanted me to sign a form saying I was knowingly putting my child and other children at risk of death because I was not vaccinating. I wouldn't sign it. Anyone know what this form is all about? What repercussions should I expect since I wouldn't sign it? Thanks for the help.
