do they decide that before or after the foods are washed? Like, do they test the amount of pesticides in the middle of the apple or in the skin of the apple after it's washed or in the skin of the apple before it's washed?

They ("they" would be the USDA) test the apple after it has been handled the way they expect a consumer to handle the products. Like, they wash the apples, peel the oranges, etc. There have also been a lot of tests done to see how much of the poison gets into the flesh of the fruit/veggie. The over all score is based on how much is left in the whole item afterwards. here's the link I got that from
