I'm filling out papers to submit to my insurance in an attempt to get a reimbursement for my HB last year. ( a bit late I know...but hoping )<br><br>
I can't find anywhere online that has the codes for place of service, procedures, services, or supplies, diagnosis, etc. I'm using the standard 1500 form at nucc.org.<br><br>
Any help for me? I'm looking specifically at lines 21 and 24.<br><br>

Was it a home or hospital birth?<br>
Are you billing for complete prenatal care or just the birth ?<br>
Complete PN care is >6 PN visits. If there were less than that they are billed separately and have to be coded as such.<br>
If you have a fax, I could fax you a completed sample form.<br>

It was at home...and less than 6 prenatal visits.<br><br>

Can anyone help out? I still need help. A sample form with codes would really be wonderful.

If you did and you are still having a problem...we should go over it # by #.<br>
What is the code you need and what part of the form are you having a problem with.<br>
All the boxes do not need to be filled in....some of them don't have anything to do with OB billing...<br>
Sorry if it is confusing...I do it on a computer billing program and send the claims electronically, but I can print out another one and send it to you....or we could talk on the phone.<br>

so -- this is the coding that has gotten me payment most of the time:<br><br>
line 21<br><br>
1. V22.1 (or V22.0 for a first pregnancy)<br>
2. 650<br>
3. v24.1<br><br>
then line 24<br><br>
The prenatal section. The code is for complete prenatal, as I understand it. The place code is for care at home.<br><br>
1 (dates of first prenatal -last prenatal) /12 for place of service/9 for type of service/ 59426 for procedure code /1 for diagnosis code/the amount I bill for that part of the service/12 for units (or howevermany prenatals you had if they are greater than 7)<br><br>
The birth section<br><br>
2 Date(s) of birth/12 for place of service/9 for type of service/59410 for procedure code/2 for diagnosis code/the amount I bill for the birth/1 for unit<br><br>
The postpartum section<br><br>
2 (dates of first postpartum-last postpartum/12 for place of service/9 for type of service/59430 for procedure code/3 for diagnosis code/amount I bill for postpartum/3 (or howevermany postpartums you had)<br><br>
I don't guarantee that any of this is right and would in fact welcome criticism from other midwives, but I have had a number of bills paid with this coding.<br><br>

I'm about ready to give up on trying to send this in. April 1st will be a year since ds was born and it appears I still need to send this to the mw to sign too. <img alt="" class="inlineimg" src="/img/vbsmilies/smilies/dizzy.gif" style="border:0px solid;" title="Dizzy">: I'm guessing it's probably too late anyway.<br><br>
*sigh* shoulda thought about doing this a long time ago and just didn't. I didn't realize we had gone over our out of network deductible is WHY I hadn't thought about doing this. :/<br><br>

Hi...I'm sorry, the fax went through the maching.....grrrrrrr.<br>
I will be in my office tomorrow to drop something off before leaving town.<br>
