You want your OB or midwife to sign off on it, including any others in their practice who might see you for the birth. You want them to have a copy in your files, and you also want the hospital L&D unit to have a signed copy in advance as well. Usually when you pre-register you can bring a copy with you and request that the nurse place it in your files. You may also want to bring copies with you in person *just in case*.

we have my OB sign it, so that if some other dr. is on call, they will still honor the birthplan she has approved.
she sends a copy to the hospital for my file there, keeps a copy in my file at the OB office, and I keep a copy to take along with me in my labor bag.
