no, you need the conscientious objection form for lots of schools, and even preschools and day cares.
I don't know specifically of drs in WA, and many doctors refuse, but as I understand it, they have a legal obligation to give the form. the form isn't saying that the Dr condones non-vax (though most think of it that way). quite the opposite - it's really a form stating that the Dr has given you all the information regarding vaxxing and you have gone against medical advice and are refusing, for whatever reason. it's a really simple form to get sorted in most cases.

 

I spoke with someone who volunteers with the AVN and she said that technically the schools aren't allowed the demand the form, but they do anyway and we then have the choice to rock the boat and refuse or to go along with it and submit the form. So, it's a pain really...either we tick off the new school staff or we jump through their hoops...

 

Wow, in North Queensland nobody in the public school system has ever asked about my kids vax status. Never.

We do delayed selective vax, & I have filled in various different things on the enrollment forms. It might be different now that our oldest is attending a private school this year, but I just fill in the same thing on all the application forms... N/A, does not apply, not sure.....

Maybe you need to pick a different state to migrate to....

 

No need for a form in NSW, you just need to provide an immunisation record - it can be blank!
CO form is just for more than the base rate of centrelink benefits and child care benefits

DD1 started in last year in Y2 and we haven't been asked for any form. On the school info form I simply left the 'vaccinations completed' and 'GP' details blank, and they didn't say anything. When I filled in the form for this year, it had last year's details in, with the vaccinations completed as "no". They never asked for anything. We're in Canberra, public school.
When we did a short stint at school in Brisbane a couple of years ago, they never asked for anything either.
