You will probably be fine with a large. I'm a size 10-12 and I wear a small. They run large.

 

I think a large will be just fine---you could probably even use a medium! After ds was born and my milk came in, my chest (incl. bust) measured 42" and I still had lots of tail with my medium.

I was about that size and used a medium. I wish now I'd gone for the extra length though.

Now that DD is 5 and weighs too much for it as a sling, I've taken the rings off and used it as a rebozo, on both shoulders. The extra length for tying a good knot would have been nice.

 

I am 5'5", size 24-26, wear a 44-46 E or so bra, and I find the medium Maya fits great and gives me lots of tail. I think large will work too... longer tail wouldn't interfere with wearing imo.

My best friend is 5'5" 190ish and a 40DD and wears a medium very well with lots of extra tail room for adjusting.

 

I'm 5'8" and a 40D, I find the large to be plenty with extra for covering over baby's head in the wind or nursing.
